01

Information and purposes

Inquiry records may contain a preferred name or identifier, pathway, safe contact method and detail, communication preferences, accessibility needs, limited routing need, deadlines, referral source, and charter information. P.R.I.M.E. uses these fields for routing, response, duplicate and abuse detection, accountability, authorized case work, and legal or safeguarding duties.

02

Sources, sharing, and processors

Information comes from the person submitting it and, later, only from authorized participants, staff, partners, facilities, agencies, or records. P.R.I.M.E. shares minimum necessary information only for an authorized purpose, valid request, legal duty, emergency, or approved processor relationship and records each disclosure when required.

03

Your choices and rights

A person may request access, correction, a copy, withdrawal of contact or disclosure permission, deletion where applicable, an appeal, and a list of relevant disclosures. Rights requests must be authenticated through a secure and reliable method before records are changed or released.

04

Washington consumer health data

If Washington's My Health My Data Act applies, P.R.I.M.E. will provide the required conspicuous policy, consent and separate sharing consent where required, withdrawal, authenticated access and deletion, processor controls, appeals, and backup deletion timing. The public intake remains gated until counsel makes the applicability determination.

05

Security and retention

Staff access requires unique accounts, verified MFA enforcement, completed training, current access review, least privilege, restricted records, immutable audit events, controlled exports, legal holds, incident response, backups, restore testing, and counsel-approved retention and deletion rules.

06

Never submit publicly

Do not submit Social Security or bank numbers, passwords, identity documents, detailed clinical or treatment records, victim names or addresses, graphic offense narratives, or complete court and corrections files through public intake.
